Care Coordinator
Care Coordinators - Seeking full-time Care Coordinators. RN, BSN preferred with a minimum of one year of progressively more responsible or expansive experience. These positions will be responsible for care coordination of designated persons served, including active engagement, sharing clinical information, and coordinating services and supports amongst providers. EOE. Starting salary $55,665 to $64,015 dependent upon experience and credentials/licensure.
